Coventry and Warwickshire Children’s Rights Service Young Person/Family Member Enquiry Form

The young person being referred needs to be aged 5 to 25 years old, needs to have a Coventry or Warwickshire Social Worker. The advocate will then speak to the young person/family member as to whether their situation meets our criteria.
